3 Easy Ways to Send a Case



1. Send an IOS file to Biogenic Dental Laboratory



1. Call Biogenic Dental Laboratory at (800) 367-3322 for instructions on sending your .stl file.

2. Ship your case to us with US Mail or UPS.

To Mail a Case via the US Postal Service or UPS:

  1. Download and fill out the Biogenic Doctor Rx Form. with your case information, including:

    • doctor name
    • practice address
    • email
    • phone and fax number
  2. Request a Mailing Label for the Best Shipping method for my location.
  • For future cases, we can supply you with extra Biogenic Dental mailing boxes and labels.
  • To have supplies faxed call 1-800-367-3322 and ask for Laurie or Melissa.
